Hi,
What does exactly mean "+CREG: 4" (or "+CREG: 0,4")?
The els61-e2_atc_01000.pdf says "Unknown (not used)" but that can't be true since the module is spitting it out. On the contrary, the els61-us_atc_v01000c.pdf says "Unknown, e.g. out of UMTS/LTE coverage."

There are statuses defined for all the common situations. This 'Unknown' status should be interpreted as non of the other defined statuses. So it should be rather uncommon to happen. At least I probably have never seen it. Can you describe in what circumstances it happens to you?

I've seen it many many ***** on several modules in several different circumstances. But if I have to point to something specifically it kind of tends to happen more frequently when turning on the modem again after it was sent an AT^SMSO command.
So something like:
- modem ON
- registered and connected
- successfully managed some communication
- after some time an AT^SMSO is sent
- wait for the ^SHUTDOWN
- wait for up to 1 second(*) to see the V180 going low
- some time later (eg 5 seconds) turn on the MODEM again
- we may get +CREG: 4, when it happens we stay unregistered for minutes until we hit our timeout and we turn off everything.
(*) Note that we have seen ***** when we wait for longer the 2 seconds but the V180 is still high! After 1 second regardless of the V180 we turn off the power to the modem. The +CREG: 4 happens regardless of the V180 status, meaning that I've seen many ***** the V180 low after 250ms and at the following power up the modem reports +CREG: 4.

Cutting off the power before V180 goes low is not recommended. There is some small risk of causing module damage - after SMSO it performs some actions and when the power is cut off while some write operation on flash is pending the result may be unpredictable.
And have you checked AT+CEREG? when this CREG status happens? CEREG should be used for LTE instead of CREG. Please also check ATI1 reply for this module.

The documentation says "Indicates that the power-off procedure is finished and the module will be switched off in less than 1 second." and based on this would you expect the V180 the go low after 1 second? If yes, what would you suggest us to do if the V180 doesn't (eg several seconds and still high)? For power consumption reason we have to pull the plug at some point...
We are not currently using CEREG, and by the way how I am supposed to choose between CREG and CEREG when I am in dual mode LTE/GSM?

After SMSO the module performs some operations and throws '^SHUTDOWN'. To speed up you can use fast shutdown. V180 should go low after '^SHUTDOWN' URC in less then 1 second (in case of fast shutdown there is no URC). Is it always longer in your case or sometimes only? Did you test the maximum time? I think that it could also be related to the hardware with which the module is used - maybe the line is back powered somehow.
As for CEREG I think that you could use both or verify COPS or check CEREG in case the module is not registered according to CREG.

It is not everytime, some ***** it case as little as 250ms.
I've seen V180 staying high after the URC for more than 2 seconds many *****.

If after '^SHUTDOWN' URC is thrown V180 does not go low in less then 1 second and it happens on a regular basis I think that you may report it to Gemalto support. It may (or not) be related to your hardware. But anyway additional tests with cooperation with support team and module traces may be required to investigate the cause of this behavior.
